Employee benefit forms and information can be found on the Business Department's page on Moodle--do a search for the Business Department course. (To sign into Moodle, your user id is the first part of your email address, i.e. without the @wsdr4.org. The password is the same as your email password.)
Please note, this option is available to those whose eligibility was sent in and is loaded into the Anthem system, but may not have received their ID card yet. This can be especially helpful if you are a new hire or have plan option changes.
